First Step-After the initial call from a potential client, we suggest scheduling a home & patient assessment. The purpose is to assess the home condition and the patient's needs. It usually takes 45min to 1 hour to complete. A Case Manager conducts the assessments. The fee is $125.00.

• Patient assessment: Conduct vital signs, physical abilities, and medication documentation

• Plan of Care: Personalized plan of care designed, assign a caregiver, and services scheduled. 

• Home Assessment: Complete assessment and home safety walk-through.

• Home Adaptation and Cleaning: adaptive equipment recommendation, room reorganization, and house cleaning schedule
