Care Manager Responsibilities:

• Creating and updating personalized care plans for clients.

• Manage Nightingale Wings, LLC client and caregiver registry.

•  Conduct comprehensive assessment of patients’ physical, mental, and psychosocial needs.

•  Collaborating with clients' families, friends and social supports in developing treatment plans.

• Identifying and recruiting high-quality healthcare providers.

• Behavioral strategies including motivational interviewing and self-management support.

•  Interacting with healthcare providers to facilitate clients' treatments.

•  Ensuring clients’ treatment requirements are met.

• Suggesting alternative treatment plans when clients' services requests do not meet medical
  necessity criteria.

•  Instructing and educating clients on procedures, healthcare provider instructions and referrals.

•  Linking clients to social services programs and entitlements such as transportation assistance
  and translation services.

• Follow-up with clients within 24 hours on inpatient discharge & within 48 hours of ED visit notification

• Conducting regular follow-ups with clients to evaluate progress, promote continuity of care and
  ensure improved health outcomes.

• Maintaining records of care management activities

Required:

• Background check

• Authorized to work in the U.S.

• Reliable Transportation

• Bachelor’s degree in social sciences, social work, nursing or a related field.

• 3+ years work experience in public health or related field.

• 1+ years current case management experience.

•  Registered Nurse (RN) license

• Certified Case Manager (CCM) license preferred.

• Proficient with Healthcare Management Systems and Microsoft Office Word and Excel.

• Outstanding communication skills, both written and verbal.

• Excellent organizational skills.
